CurrentCare gives you secure, web-based access to your enrolled patients’ most up-to-date integrated data with the CurrentCare Viewer. In addition, CurrentCare Hospital Alerts use Direct messaging to notify primary care providers when their consented patients have an inpatient, outpatient, or emergency department encounter at a participating hospital. To become a CurrentCare enrollment partner:
1) Download and complete a CurrentCare Letter of Agreement and a W-9 form.
The Letter of Agreement provides us with information about your practice. The IRS W-9 Form (Request for Taxpayer Identification Number and Certification) is needed to receive the $3 per patient subsidy payment.
2) Sign and send the two documents in Step #1 to the Rhode Island Quality Institute by email (CurrentCare@riqi.org) or by fax (401-276-9144).
3) Meet with a CurrentCare Community Outreach Specialist.
This specialist will help you complete a CurrentCare Enrollment Goal Worksheet, train your staff, and provide information about incorporating enrollment in your practice’s workflow.
